Forms

Prior to your first office visit, download the forms and documents on this page.  Complete them to the best of your ability, date and sign.  Thoroughly read the Policies and Practices notice.  Bring everything with you to your first appointment.  We will discuss all forms at that time.

Please arrive 15 minutes prior to your first appointment to go over paperwork.

Please bring your driver's license (or passport) and insurance card (if applicable) to your first session.
